Federal Contractors: New Form for Voluntary Self-Identify as an Individual With a Disability

Overview.  As you know, beginning March 24, 2014, the Office of Federal Contract Compliance Programs (OFCCP) has required federal contractors to provide workers with the opportunity to voluntarily identify as an individual with a disability. Federal contractors need this information to measure their progress toward achieving equal opportunity for people with disabilities. The invitation to self-identify must be extended in the form approved by the Office of Management and Budget (OMB).

The original form expired on January 31, 2017.  A new form has been issued with an expiration date of January 31, 2020. No substantive changes have been made to the form, only the expiration date.  Download the updated form here.

Call to Action.  Federal contractors must begin using the form with the January 31, 2020 expiration date immediately.
